Advanced Heart Failure Physician

Provide advanced heart failure patient care within the practitioner’s scope, participate on Medical Staff Committees, ensure licensure and credentialing requirements are met, and adhere to safety standards. Must be eligible to bill under Medicare/Medicaid; orientation and ongoing competency assessments are required. Desirable board certification in the specialty. Minimum qualifications include board eligibility at initial appointment and active state licensure in Kentucky/Indiana; completion of an ACGME-accredited residency.
