Advanced Heart Failure Physician

Physician HTPN examines and treats patients with heart failure, takes patient histories, requests tests, makes diagnoses, and prescribes treatments. They administer vaccinations, perform follow-up exams, and educate patients on disease prevention and health maintenance. They supervise the clinical activities of a care team that may include technicians and advanced practice professionals. Responsibilities include examining and recording patient information, prescribing or administering treatment and vaccines, ordering and interpreting tests, collaborating with a clinical team, monitoring patient progress, diagnosing and treating conditions in the heart failure specialty, and creating post-operative care plans with lifestyle recommendations. The role may be based in a hospital, specialty clinic, or outpatient surgical center, and involves supervising care teams as part of an integrated clinical workflow.
