Advanced Foot Care Nurse (Home Care) - RN or RPN

Job Summary
Conduct comprehensive foot assessments, develop individualized care plans, and provide routine and advanced foot care services including nail trimming, thickened nail management, callus reduction, and diabetic foot assessments. Perform basic skin and foot assessments, educate clients on foot health and self-care, and identify abnormalities requiring referral to physicians or podiatrists. Monitor clients for condition changes, communicate concerns appropriately, and maintain accurate documentation within AlayaCare while obtaining informed consent. Collaborate with physicians and interdisciplinary providers, adhere to infection prevention standards, and follow College of Nurses of Ontario Standards of Practice. Work independently in client homes throughout the Greater Toronto Area, Monday to Friday with weekends as needed, earning $65.00 per hour.
Required Qualifications
- Current registration in good standing with the College of Nurses of Ontario (RN or RPN)
- Valid Advanced Foot Care Certificate from a recognized educational provider
- Previous experience providing foot care within community or home care settings
- Knowledge of diabetic foot care and risk assessment
- Strong assessment, clinical judgement, and critical thinking skills
- Excellent interpersonal and communication skills
- Comfortable working independently in clients' homes
- Basic computer proficiency and ability to document electronically
- Current CPR certification
- Valid Ontario driver's licence (G2 or G) and access to a reliable vehicle
- Monday to Friday
- Weekends as needed
- Day shifts

Desired Qualifications
- Experience working with seniors and individuals living with chronic diseases such as diabetes or vascular conditions
- Knowledge of the Canadian Association of Foot Care Nurses (CAFCN) National Competencies for Advanced Nursing Foot Care
- Previous community nursing experience
- Experience using AlayaCare or other electronic health record systems
