Job Summary

Perform comprehensive primary health care services including history and physicals, diagnostic ordering, and treatment initiation for new and returned clients. Manage acute and chronic illnesses per established protocols, provide follow-up referrals, and document all patient activities while maintaining mandated annual encounter levels. Requires a Master's degree, active IL license, and fluency in Spanish or ability to utilize interpretation services. Full-time role with competitive salary, merit increases, and 20 days PTO.

Required Qualifications

  • Master of Medical Science or M.S. in Physician Assistant Studies or an MSN from an accredited program
  • Active PA or APRN license in IL or eligible for IL licensure
  • Board Certification
  • Active DEA and Controlled Substance Licenses or eligible for DEA and Controlled Substance Licenses
  • Active CPR (BLS) certification
  • Minimum of two (2) years in primary care practice
  • one (1) year of practice as an FQHC primary care provider
  • completion of an APP primary care Fellowship program post-graduation
  • Ability to utilize interpretation services for patient encounters

Desired Qualifications

  • DNP from an accredited program
  • Experience serving in an FQHC, Community Health Center, or working with traditionally underserved patient populations
  • Fluency in Spanish or a language in addition to English
  • Knowledge of or interest in learning medical Spanish
  • Commitment to increasing number of APRNs working in underserved communities and providing high-quality compassionate patient care
  • Comfort with providing clear and constructive feedback
  • Energetic, deadline-driven, self-starter
  • Excellent organizational and project management skills
  • Working knowledge of PC using Microsoft products such as Word and Power Point in a Windows operating environment
  • Ability to develop and maintain excellent working relationships with preceptors, partners, clinical staff, managers, and directors
  • Interest in and comfort with working with diverse patient population
