Job Summary
Independently assess, diagnose, and manage patients with acute, chronic, and complex conditions across home visiting, care home, and population health services. Develop evidence-based treatment plans, prescribe medications where qualified, and interpret investigations while delivering holistic care in community settings. Collaborate with GPs and multidisciplinary teams to contribute to service development, quality improvement, audit, and clinical governance. Provide clinical leadership and mentorship to colleagues within a multidisciplinary team. This role offers 20 hours per week with peripatetic coverage across multiple Lewisham sites for an Advanced Clinical Practitioner with an MSc and independent prescribing qualification.
Required Qualifications
- Registered healthcare professional with current UK registration (NMC or relevant professional body)
- MSc in Advanced Clinical Practice (or equivalent Master's level qualification)
- Independent Non-Medical Prescriber (where applicable)
- Minimum one year's experience working within General Practice
- Experience using EMIS clinical systems
- Excellent clinical assessment, diagnostic and decision-making skills
- Strong communication, leadership and teamworking abilities
- Commitment to evidence-based practice and continuous professional development
- Location: Lewisham Borough (Peripatetic across multiple sites)
- Hours: 20 hours per week
Desired Qualifications
- Three or more years' experience in General Practice
- Experience leading clinical services or quality improvement projects
- Experience working within Lewisham healthcare services
- Knowledge of digital health transformation
- Experience supervising or developing other clinicians
