Job Summary
Provide primary care to high acuity patients in their home Monday through Friday, focusing on patient needs rather than volume. Deliver in-home primary care services for individuals with complex healthcare needs under an integrated, physician-driven model. Assist with appointments in select markets where available, with minimal call responsibilities and no weekends or evenings. HarmonyCares operates home-based practices in 14 states, employing over 200 providers to bring personalized care to patients who have difficulty accessing traditional healthcare.
Required Qualifications
- Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
- Federal DEA License/eligibility
- Active CPR/BLS Certification
- Board Certification

- Geriatric training/experience preferred
