Job Summary
Initiate and facilitate advance care planning conversations tailored to individual patient needs, health status, and values. Assess patient requirements for legal healthcare directives and document treatment preferences to support future decision-making. Collaborate with Clinical Ethics, Oncology, and Primary Care teams to promote ACP as a standard component of system-wide healthcare initiatives. Develop and deliver staff and community education programs while maintaining current knowledge of evidence-based practices. Travel regularly to multiple clinics across the Indianapolis area to meet patients at various stages of their care journey. Requires a valid Indiana license as a Social Worker, Chaplain, or Registered Nurse, along with strong emotional intelligence for navigating sensitive, non-scripted discussions. Full-time day shift at Arnett Hospital in Lafayette, IN.
Required Qualifications
- Bachelor's Degree
- 5-7 years of experience in the healthcare field
- Requires licensure or board certification within two years of hire
- Valid Indiana license as a Social Worker, Chaplain, or Registered Nurse with an active RN license in Indiana or the Nurse Licensure Compact (NLC)
- Reliable transportation for regular travel between clinic sites
- Travel to multiple clinics across the Indianapolis area
