Job Summary
Initiate and facilitate advance care planning conversations tailored to individual patient needs, health status, fears, concerns, and beliefs. Assess patient requirements using a person-centered approach to document legal healthcare directives reflecting their values and preferences. Support program development and system-wide implementation of ACP initiatives across IU Health's 16 hospitals. Collaborate with Oncology, Clinical Ethics, and Primary Care teams to align care with patient goals. Develop and deliver staff and community education programs while maintaining current knowledge of evidence-based practices. Travel regularly to multiple clinics throughout the Indianapolis area to meet patients where they receive care. Maintain a valid Indiana license as a Social Worker, Chaplain, or Registered Nurse. Full-time day shift with reliable transportation required for inter-clinic travel.
Required Qualifications
- Bachelor's Degree
- 5-7 years of experience in the healthcare field
- Requires licensure or board certification within two years of hire
- Valid Indiana license as a Social Worker, Chaplain, or Registered Nurse with an active RN license in Indiana or the Nurse Licensure Compact (NLC)
- Reliable transportation for regular travel between clinic sites
- Travel to multiple clinics across the Indianapolis area
