Adult Therapist / Medical Case Manager

Job Summary
Provide intake screening and mental health assessments; identify service needs and recommend options for care. In collaboration with agency psychiatrists and medical secretary, deliver re-assessment and treatment planning services, brief individual crisis intervention or supportive counseling, symptom management skill-building groups, and case management or community support for adult clients receiving medication treatment. Conduct individual, group, and family therapy to address functional impairments; develop treatment plans with clients and involve them in setting goals. Provide face-to-face re-assessments (at least annually for meds-only clients) and treatment plan reviews (at least every 6 months). Deliver brief crisis intervention, stabilization services, and consult with psychiatrists on clinical issues. Provide back-up coverage for Medical Secretary and coordinate care with internal and external providers. Ensure timely clinical documentation and service reporting; participate in supervision and staff development; work cooperatively with Center teams. Maintain familiarity with funding sources and billing requirements. Schedule is Full Time, Exempt, 37.5 hours per week with potential for evenings and Saturday hours; Equal Opportunity Employer.
Required Qualifications
- Masters degree from an accredited college or university in Social Work, Psychology, or closely related discipline and be licensed or eligible for certification
- Must have supervised experience in providing counseling and/or psychotherapy services
- Knowledge in diagnostic assessment, long and short term treatment, family systems theory and group treatment
- Basic computer and keyboarding skills
