Job Summary
Conduct assessments, including bio-psychosocials and comprehensive psychosocials, within 30 days of intake and finalize documents in Avatar within 24 hours. Perform home visits to evaluate client safety and well-being, maintaining at least 85% compliance with visit requirements. Develop and update individualized service plans with clients and guardians, setting measurable goals and objectives. Assist clients in accessing financial, medical, social, and educational services while monitoring case management productivity standards. Document all services provided in client records, ensuring progress notes reflect service goals and client progress. Identify high-risk clients, complete suicide risk assessments, and implement personal safety plans. Maintain current ADM Targeted Case Manager Certification and a valid driver's license.
Required Qualifications
- Certified Behavioral Health Case Manager (CBHCM) verified through the Florida Certification Board (FCB) in accordance with Florida Statute 394.4573(e)
- Bachelor's Degree in counseling; Social Work; Psychology; Criminal Justice; Nursing; Rehabilitation; Special Education; Health Education; or a Human Services Field in which major coursework includes the study of human behavior and development
- Current valid driver's license
- Maintain Apalachee Designated Auto Driver status
- Maintain current ADM Targeted Case Manager Certification
Desired Qualifications
- Must obtain Certified Case Manager – obtain Provisional Status (CBHCM-P) within 3 months of hire
- Other Bachelor's degree from an accredited university or college with three years full-time or equivalent experience working with adults experiencing serious mental illness
