Job Summary
Urgent social work assessments for adults to determine eligibility for care under the Care Act 2014, address immediate needs and coordinates rapid response services. Responsibilities include rapid response assessments, crisis intervention to prevent hospital admissions and support timely discharges, safeguarding and multi-agency coordination with health professionals and care providers. Qualifications include Social Work England registration, eligibility to work in the UK, a full UK driving licence, post-qualified experience, and a degree level or equivalent in Social Work. Hybrid working arrangement for a 3-month contract.
