Job Summary
Adult Social Worker in the Response Team based in Portsmouth. Core duties include triaging referrals, providing information and advice, signposting to the voluntary sector, completing Care Act assessments and creating support plans, sourcing care packages, triaging SCAS referrals, conducting safeguarding enquiries, and supporting community visits to ensure individuals receive needed care. Requires Social Work England registration, a Social Work degree, and at least 2 years post-qualifying experience; hybrid working arrangement with office-based 2-3 days/week and flexibility for community visits.
