Job Summary
Provide client-centered rehabilitative services and teaching skills to individuals with serious mental illness or traumatic brain injury, enabling recipients to develop psychiatric stability and independent living abilities. Deliver instruction, support, and self-advocacy training while monitoring medication and managing symptoms. Coordinate services with agencies, employers, and families to promote community reintegration and complete individual treatment plans within 30 days of onboarding. Maintain strict confidentiality, document concurrent notes before client discharge, and participate in required supervision and agency meetings. Achieve productivity expectations and complete annual Relias Learning trainings. Travel between facilities as needed and support new team members with client information training.

- Mental Health Practitioner Status obtained through education and/or experience
