Job Summary
Provides assessment and one-on-one case management for elderly and disabled adults at risk of neglect, abuse, or exploitation. Conducts assessments of physical appearance, hygiene, medical/nutritional/environmental needs, competency and self-determination, activities of daily living, and financial management; seeks protective services and guardianships in emergencies; collaborates with law enforcement and district attorney’s office on investigations; coordinates services with multiple community agencies; develops and implements service plans; attends inter-disciplinary team meetings and mandatory continuing education; conducts outreach and education about adult protection, and documents all assessments and actions clearly. Requires a bachelor’s degree in a human behavior science field (or equivalent coursework) and one year of professional casework experience; bilingual English/Spanish is desired. Valid driver’s license required and background/education verification apply. The role is office-based with potential exposure to health and safety risks; offered as a full-time position with promotional opportunities within the Social Case Worker levels.
Required Qualifications
- Bachelor’s degree from an accredited institution with a major in a human behavior science field, OR a degree with 30 semester hours, or 45 quarter hours, of qualified work in development in human behavioral sciences or health care related courses such as social work, sociology, psychology, psychiatry, gerontology, nursing, special education, family intervention techniques, diagnostic measures, therapeutic techniques, guidance and counseling, criminal justice, or other human behavioral sciences, or a medical field relevant to the APS program
- One year of professional casework in a public or private social services agency completed after the degree is obtained
