Adult Patient Navigator (Full-Time)

Job Summary
Coordinate care between patients and service providers within Crawford County Mental Health Center and the community to address social determinants of health. Accurately obtain and record patient National Outcome Measures (NOMs) information and vitals in the EHR while assessing client needs including demographics, historical data, BMI, and Columbia. Provide outreach via telephone and travel to assist clients with access to health services, eliminate barriers, and arrange medical, social, and educational referrals. Maintain a caseload of up to 450 clients, complete hospital admission and discharge outreach within 24 hours, and track referrals for primary care, labs, and screenings. Obtain MOAB certification for managing aggressive behavior and First Aid CPR training. Assist clients in applying for financial assistance and Medicaid.
Required Qualifications
- Graduation from high school or equivalency
- A valid driver's license
- Pass pre-employment background checks
- Required to follow dress code policy of any alternative work locations, including but not limited to schools, jails, courts, detention centers, other community partners, etc.
- Ability to move freely in the community and in a wide variety of circumstances
Desired Qualifications
- A bachelor's degree from an accredited university in the various behavioral health fields such as clinical psychology, social work, counseling, nursing, rehabilitation or closely related degree
- Experience in helping professionals and occupations
