Job Summary
Family Nurse Practitioner role in Middletown, NY serving residents in assisted living facilities. Responsibilities include conducting comprehensive health assessments (physical examinations, medical histories, medication reconciliation), developing and implementing individualized care plans in collaboration with the interdisciplinary team, and managing chronic conditions and acute illnesses with timely interventions and referrals as needed. Requires NY NP license and DEA license to prescribe commonly prescribed medications; Master’s degree in Nursing; eligibility to provide reimbursable services and maintain provider status with Medicare/Medicaid. Schedule is Monday–Friday, 8am–5pm with no nights or weekends. Compensation approximately $140,000+ (W2 or 1099 option available), full benefits, PTO, sick time, CME reimbursement, 401k with match, and life insurance. Required Qualifications
- Master’s degree in Nursing and graduate of an accredited Nurse Practitioner program
- Unrestricted New York State Nurse Practitioner license
- Unrestricted DEA license to prescribe all commonly prescribed medications
- Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid
