Job Summary
Conduct in-home patient visits to assess clinical status and provide post-discharge medical care for patients transitioning from acute facilities. Review discharge summaries and progress notes, then collaborate with home health professionals and the care team to coordinate seamless treatment. Provide medical consultations, prescribe medications when appropriate, and oversee ongoing care plans to promote recovery and reduce readmissions. Requires a current Texas medical license, active Medicare provider number, and Nurse Practitioner certification in good standing. Ability to travel within a 35-mile radius of home is essential. Base salary ranges from $115,000 to $135,000 with quarterly performance bonuses available.
