Job Summary
Adult Home Care Coordinator provides intensive care management and transitional support to adults identified as AH class members under relevant settlements, functioning as a member of an interdisciplinary team to facilitate transition from adult home to community. Responsibilities include evaluating transition interest, educating on community-based benefits, referring to the Housing Contractor for in-reach, enrolling in Health Home when transitioning or continuing care, completing the Discharge Planning Tool, coordinating pre/post-transition calls, maintaining monthly face-to-face contacts, conducting telephonic updates, reviewing identification/benefits needs, assisting in the movement to community, supporting ongoing skill-building, documenting Health Home Core Services, and participating in disaster preparedness activities. Requires a bachelor’s degree in a related field or equivalent licensure/certification, plus two years of relevant experience; master’s degree may substitute for one year of experience. Must be able to travel via public transportation and perform physical tasks as needed.
Required Qualifications
- Bachelor’s degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing, or other human services fields
- NYS teacher’s certification (bachelor’s degree required) or NYS licensure and registration as a Registered Nurse with a bachelor’s degree or
- Bachelor’s level education or higher with five years of experience working directly with persons with behavioral health diagnosis
- Credentialed Alcoholism and Substance Abuse Counselor (CASAC)
- Two years of direct service experience with persons with Serious Mental Illness, developmental disabilities or substance use disorders
- Experience substituting a master’s degree for one year of experience
