Adult CCS Care Coordinator

Conduct comprehensive strengths and needs assessments during intake and throughout services, then develop and implement individualized Recovery Plans of Care. Coordinate team meetings with service providers and support networks while providing crisis intervention, safety planning, and advocacy for court hearings and psychiatry appointments. Safely transport clients to appointments and build strong formal and informal support teams. Requires a Bachelor's degree in Social Work, Psychology, Criminal Justice, or related field, a valid driver's license, and local travel with mileage reimbursement.
