Job Summary
Conduct adult case management for individuals with intellectual disabilities and autism across Western Maine, identifying needs, developing care plans, and coordinating referrals. Monitor service effectiveness, advocate for identified services, and ensure individual health and safety. Work closely with multi-disciplinary teams to support service satisfaction and choice. Maintain a reliable vehicle with proper insurance and complete state-mandated training programs. This hybrid role offers supervision for LSW licensure and is part of Waypoint's growing network providing residential, in-home, and therapy services.
Required Qualifications
- BA/BS in human services or closely related field
- BA/BS in unrelated field + minimum 1 year experience in adult IDD services
- Reliable vehicle with proper insurance
- Valid driver's license issued by state of residence with clean driving record
- Proficiency in speaking, reading, and writing English
- Strong interpersonal and communication skills
- Ability to complete agency and state mandated training programs with proficiency
