Job Summary
Adult Case Manager (West Valley) provides ongoing community-based case management for Seriously Mentally Ill (SMI) members, including transportation coordination, coordination of behavioral and physical health services, and liaison with the Clinical Team. Responsibilities include arranging and coordinating services, ensuring documentation and communication across the team, handling transitions to adulthood referrals, understanding patient rights regarding access to treatment, appeals, and complaints, coordinating with primary care services, performing out-of-office and administrative tasks per agency procedures, and supporting bilingual Spanish-English needs as applicable.
Required Qualifications
- Minimum High School Diploma
- quality experience in the behavioral-health field
- Arizona Driver's License
- Valid Auto Insurance
- MVR matching JFCS driving guidelines
- Reliable Transportation
