Job Summary
Adult Case Manager (Part-Time) provides in-home and community-based behavioral support to adults with mental illness. Responsibilities include developing leisure and home maintenance skills, orienting clients to community resources, helping build social connections, setting boundaries and structure, educating on healthy living, coordinating with the primary clinician and staff, reviewing goals, and contributing to performance evaluations. Administrative duties include timely documentation and participating in required trainings and meetings. Work primarily in the community with occasional office time and driving to client homes; schedule includes evenings and Saturdays. Opportunities for growth into other clinical roles and continuing education.
Required Qualifications
- Associates degree or equivalent from an accredited college or university in social services
- Previous experience working with adults
- Minimum age 21 years
- Ability to work evenings and Saturdays
- Willingness to drive to clients’ homes and provide services in the community
- Excellent driving record
