Job Summary
Adult Case Manager responsible for facilitating person-centered planning and delivering case management services to individuals with intellectual/developmental disability, mental health, substance use, or co-occurring disorders. Responsibilities include transportation support as required by treatment plans, developing natural supports (family, friends, religious organizations, neighbors, teachers, employers), coordinating internal and external case activities, serving as consultant to individuals and supports, documenting services per funding and regulatory standards, monitoring providers, participating in clinical discussions and ongoing professional development, and advocating for clients while linking them to additional resources. Requires communication skills, organization, and community-based service delivery experience.
Required Qualifications
- Bachelor’s degree in social work, psychology, human services, or related field required
- One year experience working with adults diagnosed with an intellectual/developmental disability or mental health disorder required
- Community-based service delivery preferred
- Knowledge of person-centered planning process preferred
- Valid Michigan driver’s license and auto insurance coverage or eligibility required
- Exceptional written and verbal communication skills
- Function in a fast-paced and ever-changing environment
