Job Summary
Conduct comprehensive assessments of medical, behavioral health, and psychosocial needs to identify risks for adverse outcomes. Develop individualized, person-centered Plans of Care and implement monitoring in collaboration with clients and interdisciplinary teams. Coordinate care following hospitalizations or emergency visits while facilitating communication among providers and community supports. Provide health education, advocate for timely access to care, and assist clients in navigating insurance benefits. Maintain accurate, audit-ready documentation and perform home or community visits as required. Bachelor's degree in Social Work or related field with 2–3 years of experience in care coordination or case management. Bilingual in Spanish preferred. Located at Manhattan office or other New York State clinics.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, Nursing, or related field
- Minimum 2–3 years of experience working with adults in behavioral health, care coordination, case management, or community health
- Strong organizational, communication, and problem-solving skills
- Ability to work independently and within multidisciplinary teams
- Experience in Medicaid care management programs
- Knowledge of serious mental illness, substance use disorders, or chronic medical conditions
Desired Qualifications
- Bilingual in Spanish preferred
