Job Summary

Maintain an active caseload with monthly member contact, conducting home, community, and office visits to support engagement and service delivery. Lead interdisciplinary team meetings, coordinate communication among providers and partners, and connect members with healthcare, housing, employment, benefits, and transportation resources. Provide comprehensive care coordination for adults in the Behavioral Health Home program while monitoring progress and adjusting care plans. Complete annual and 90-day assessments in compliance with DHHS and MaineCare requirements, maintain accurate documentation, and collaborate to address service gaps. Participate in quality improvement initiatives and serve as a liaison among families, providers, and community agencies.

Required Qualifications

  • Bachelor's degree in a related field
  • MHRT/C eligible
  • A minimum 1 year of paid experience working with individuals with behavioral health, intellectual and/or developmental disabilities (e.g., DSP, BHP, RBT, etc.)
  • Strong organizational, writing, and time management skills
  • Ability to thrive in a fast-paced environment with multitasking and problem-solving responsibilities
  • Microsoft Office Suite
  • Potential for some Hybrid work

Desired Qualifications

  • Experience working with Behavioral Health Home Services is preferred
  • Previous experience in care coordination is preferred but not required
