Job Summary
Adult Care Coordinator/Case Manager delivers client-centered services for adults with behavioral health diagnoses, developing individualized service plans to ensure benefits through the ASO Managed Care System. Requires travel within the designated area, a regular Monday–Friday schedule between 6am–5pm, and possession of a valid Maine driver's license with adequate vehicle insurance. Strong computer skills, MHRT/C or MHRT/CP credentials, and a high school diploma or equivalent are required. Includes mileage reimbursement, flexible schedule, paid travel time, holidays, and comprehensive benefits; office-based role with travel within Windsor, ME.
Required Qualifications

- Proof of vehicle insurance at $50K/$100K and a valid Maine driver's license
