Job Summary
Adult Care Coordinator/Case Manager focuses on adults with behavioral health diagnoses, developing client-centered service plans to ensure services are approved through the ASO Managed Care System. Role requires traveling within the designated area, maintaining benefit information for clients with cognitive disabilities, and coordinating care across settings. Must hold MHRT/C or MHRT/CP, have a valid Maine driver’s license and vehicle insurance ($50K/$100K), be at least 18, and demonstrate intermediate computer skills. Offers mileage reimbursement, flexible schedule, paid travel time, holidays, and team-based environment support including professional supervision and access to agency benefits (life, disability, health, and equipment). The position adheres to standard office hours Monday through Friday, with travel between 6am and 5pm in a regular schedule.
Required Qualifications
- MHRT/C or MHRT/CP
- High school diploma or equivalent
- Proof of vehicle insurance at $50K/$100K

- Intermediate level computer experience
- Five days per week Monday-Friday, travel in designated area
