Job Summary
Adult Case Manager provides services for adults with behavioral health diagnoses, developing client-centered individual service plans to ensure services are approved through the ASO Managed Care System; maintains benefit and financial information for clients with cognitive disabilities; works a regular office schedule Monday–Friday 6am–5pm with travel within the designated area; requires vehicle insurance, a valid Maine driver's license, and a high school diploma or equivalent; duties include coordinating care, assessing needs, and supporting individuals in homes and community settings; benefits include mileage reimbursement, flexible schedule, paid travel time, holidays, life insurance, disability coverage, and provided equipment.
Required Qualifications
- High school diploma or equivalent
- MHRT/C or MHRT/CP
- Intermediate level computer experience
- Valid Maine driver's license
- Proof of vehicle insurance at $50K/$100K
- Must be 18 years of age or older
