Job Summary
Adult Care Coordinator/Case Manager focusing on adults with behavioral health needs. Responsible for developing client-centered individual service plans, ensuring services are approved through the ASO Managed Care System, maintaining beneficiary information for continued benefits, and travel within the designated area. Requires a valid Maine driver’s license, proof of vehicle insurance, and a high school diploma or equivalent. Works a five-day schedule (Mon-Fri) with hours between 6am and 5pm, with travel within the designated area. Benefits include mileage reimbursement, flexible schedule, paid travel time, 12 holidays, life insurance, disability, medical/dental/voluntary insurances, laptop and cell phone, clinical supervision, and trainings.
Required Qualifications
- Proof of vehicle insurance at $50K/$100K
- Valid Maine driver’s license
- 18 years or older
- High school diploma or equivalent
- MHRT/C or MHRT/CP
- Intermediate level computer experience
