Job Summary
Adult Care Coordinator provides services that focus on adults with behavioral health diagnoses and decreased ability to function independently in homes and community. Develops client-centered individual service plans to ensure services are approved through the ASO Managed Care System and maintains current benefit information for clients with cognitive disabilities. Requires travel within the designated area, a regular five-day workweek (Mon-Fri) between 6am-5pm, and possession of a valid Maine drivers license and vehicle insurance. Position offers mileage reimbursement, flexible schedule, paid travel time, 12 holidays, generous time off, and employer-provided devices and benefits.

- High school diploma or equivalent
- Valid Maine drivers license
- Proof of vehicle insurance at $50K/$100K
- Intermediate level computer experience
- 18 years of age or older
