Job Summary
ADRC Counselor responsible for professional assessment and determination of level of care and appropriateness for community-based services. Screens emergency and non-emergency referrals by phone, verifies Medicaid eligibility, educates on CCSP/HCBS, documents assessments, and identifies client needs for services. Maintains HIPAA compliance, completes required paperwork, and collaborates with team to refer clients to CCSP/HCBS or other resources. Requires a Bachelor’s degree in Social Work/Sociology/Psychology or an active Georgia RN license, two years in human services plus one year as a case manager, and a valid driver's license. Position operates in an office with local travel; hybrid schedule.
Required Qualifications
- Bachelor’s degree in Social Work, Sociology, Psychology, or a related field or a Registered Professional Nurse currently licensed to practice in the State of Georgia.
- Two years’ experience in the human service or health related field plus one year of experience as a case manager.
- Valid State Driver’s License.
- Will be eligible to take the CIRS-A/D exam after one year of experience
- Will be eligible to take Boston university Courses after becoming CIRS-A/D certified and one year of experience.
