Job Summary
ADON Case Manager (RN/LPN) leads and coordinates nursing operations, oversees Resident Assessment Instrument (RAI) and overall care plan for residents receiving skilled services, conducts interdisciplinary meetings, performs pre-admission screenings for eligibility, coordinates MDS assessments per Medicare schedule, assists with discharge planning, and supervises and supports nursing staff and CNAs while orienting new personnel in a 100% employee-owned long-term care setting in Oklahoma.
Required Qualifications
- Current and unencumbered Oklahoma RN or LPN License
- Up to date First Aid and CPR certification
- One or more years of experience in a Skilled Nursing Facility or Long Term Care environment strongly preferred
