Job Summary
Admissions/Wound Nurse (RN) responsible for initiating admission assessments for new residents and hospital returns, aiding development and direction of the Nursing Department, coordinating admission processes with the interdisciplinary team, auditing records for facility standards compliance, participating in discharge planning, reviewing and updating care plans, performing charting per policy, completing MDS and care plan documentation, and maintaining HIPAA/confidentiality. May supervise or direct resident care as necessary. Requires active NYS RN license and LTC admissions experience; strong assessment, leadership, and communication skills; proficient in MS Office.
Required Qualifications
- Active / unrestricted NYS Registered Nurse License
- Prior experience as an admissions nurse in LTC settings
- Excellent clinical assessment skills
- Ability to make good independent decisions
- Strong leadership and supervisory skills
- Computer savvy; proficient in MS Office (Excel, Word, Outlook)
- Long-Term Care Experience Preferred
- Excellent communication and interpersonal skills
