Job Summary
Coordinate with the Care Transition function to ensure new residents integrate seamlessly into the system. Receive and review inquiries via telephone, email, fax, and in person, extracting referral information to complete information sheets. Assist discharge planners, families, and physicians in arranging admissions, preparing paperwork for signatures prior to the responsible party's arrival, and guiding residents through orientation programs including room inspections and escorts. Obtain necessary identification and insurance cards, verify coverage entitlements, and complete patient costing. Process admission packets and secure signatures on all required forms within 24 hours, reporting delays to management. Provide residents with admission information packets and obtain signatures on permits, releases, and authorizations.
Required Qualifications
- High school diploma or equivalent
- Previous experience in healthcare field preferred
