Job Summary
Admissions and Care Experience Navigator Nurse coordinates admissions from intake to discharge, drives a smooth admission process, manages tours and admissions, partners with Nursing, MDS, Social Services and related departments, completes PAN Census and PASRR, arranges equipment, serves as patient/family liaison, conducts admissions and consent processes, performs initial admission assessments and medication reconciliation, and supports follow-up to ensure facility expectations are met; requires LPN/RN license in WI, CPR, 2+ years healthcare experience, English proficiency, and basic computer skills; on-site, full-time role with on-call weekend coverage in Mt. Horeb, WI.
Required Qualifications
- LPN or RN licensed in the State of Wisconsin
- Maintained current CPR certification
- Minimum 2 years of experience in healthcare, post-acute or long-term care
- Ability to relate positively, effectively, and appropriately with residents, patients, families, community members, volunteers and other staff members
- Able to read, write and speak English
- Basic computer knowledge and ability to learn company applications
