Job Summary
Review clinical referral information and respond to inquiries regarding client eligibility for hospice services. Conduct pre-assessment visits in patients' homes or healthcare facilities to provide admission services, information sharing, and emotional support. Collaborate with the Clinical Director, Intake Department, and referring physicians to verify referral appropriateness and obtain necessary certifications. Apply nursing principles to treat patients and educate families on the plan of care while participating in on-call duties as assigned. Work Monday through Friday from 11 am to 7 pm in the Cape Cod area.
Required Qualifications
- Hold current unencumbered license as a Registered Nurse
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting
- Must be computer proficient in typing and various programs, including background in EMR
- Possess and maintains current CPR certification
- Current driver's license, vehicle insurance, and access to dependable transportation
Desired Qualifications
- Bachelor's degree in nursing or health care preferred
- Hospice/home health experience preferred but not required
