Job Summary
Conduct initial hospice information sessions and admissions for patients and families entering their journey, performing clinical assessments and collaborating with physicians on eligibility. Clearly explain hospice services, goals, and benefits while completing required documentation and consent forms. Coordinate with the interdisciplinary team to ensure smooth transitions into hospice care. Schedule includes Thursdays, Fridays, and Saturdays from 9 a.m. to 9 p.m.
Required Qualifications
- Current RN license in the state of Michigan
- Minimum of 2 years clinical experience
- Valid driver's license
- Reliable transportation
- Willingness to travel locally
