Job Summary
Hospice Admissions Nurse responsible for executing the hospice admission process in compliance with federal and state regulations; advocate for the patient and family to meet their needs that hospice can cover and refer them to other resources as appropriate. Responsibilities include assessing potential patients and families, maintaining confidentiality of records, completing admissions and care orders, communicating with physicians, referral sources, patient, family, and hospice interdisciplinary team, and coordinating status updates with leadership; requires RN licensure in MS, CPR, and reliable transportation.
Required Qualifications
- Current unrestricted RN licensed in the state of operation
- 2+ years of experience in a hospice, home health, or other healthcare service delivery system setting
- Current CPR certification
- Current Driver's License and vehicle insurance, and access to a dependable vehicle or public transportation
