Job Summary
Establish and maintain positive relationships with customers and referral sources to increase access to Bristol Hospice Services in the communities served. Complete hospice consults, evaluations, and admissions by providing a complete physical assessment and history of current and previous illnesses. Facilitate the hospice referral process and coordinate with the ED, DPCS, and intake department to ensure timely admission and care coordination. Serve as a resource to review patient information, determine care needs, and provide patient/family education related to the plan of care. This role requires 20% travel and flexible work hours. Candidates must hold a current RN license with at least one year of recent professional nursing experience or a BSN.
Required Qualifications
- Must have a current RN license in the state
- Must have a minimum of one (1) year of experience as a professional nurse within the last three (3) years or have a bachelor's degree in nursing from a program accredited by the National League of Nursing
- Must be able to travel 20% of the time, often without notice
- Must be flexible in work hours
- Must possess the willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

- An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill
- Ability to work autonomously with little direct supervision
- Successfully demonstrate tact, patience and good personal hygiene
- Ability to communicate effectively
- Strong organizational skills
- Understanding of plan of care documentation
- Flexible in work hours to ensure the patients receive care as provided in the plan of care
- Ability to empathize with the needs of the ill, injured, frail and impaired
- Comfortable addressing issues of death/dying
- Calm demeanor when entering the patients home
