Job Summary
Coordinate hospice admissions by completing initial patient assessments, facilitating referrals, and determining care needs through comprehensive physical evaluations. Establish relationships with community referral sources, EDs, and agency leadership to promote care coordination and timely admission processes. Provide patient and family education on care plans while maintaining case loads as an RN Case Manager. Conduct quality assurance reviews and participate in IDT meetings to determine patient plans of care. Travel 20% of the time and work flexible hours Monday through Friday.
Required Qualifications
- current RN license in the state
- minimum of one (1) year of experience as a professional nurse within the last three (3) years
- bachelor's degree in nursing from a program accredited by the National League of Nursing
- travel 20% of the time, often without notice
- flexible in work hours
- willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

- Ability to read and follow written instructions
- An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill
- Ability to work autonomously with little direct supervision
- Successfully demonstrate tact, patience and good personal hygiene
- Ability to communicate effectively
- Strong organizational skills
- Understanding of plan of care documentation
- Flexible in work hours to ensure the patients receive care as provided in the plan of care
- Ability to empathize with the needs of the ill, injured, frail and impaired
- Comfortable addressing issues of death/dying
- Calm demeanor when entering the patients home
