Job Summary
Establish and maintain positive relationships with customers and referral sources to develop access to hospice services. Facilitate the hospice referral process by completing consults, evaluations, and admissions while providing physical assessments and histories. Assess community educational needs and coordinate with ED, DPCS, and agency leadership to promote care coordination and timely admission processes. Serve as a resource to review patient information, determine care needs, and provide patient/family education related to the plan of care. Participate in Quality Assurance Performance Improvement processes and interdisciplinary team meetings as needed.
Required Qualifications
- Must have a current RN license in the state of PA
- Must have a minimum of one (1) year of experience as a professional nurse within the last three (3) years or have a bachelor's degree in nursing from a program accredited by the National League of Nursing
- Must be able to travel 20% of the time, often without notice
- Must be flexible in work hours
- Must possess the willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

- An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill
- Ability to work autonomously with little direct supervision
- Successfully demonstrate tact, patience and good personal hygiene
- Ability to communicate effectively
- Strong organizational skills
- Understanding of plan of care documentation
- Flexible in work hours to ensure the patients receive care as provided in the plan of care
- Ability to empathize with the needs of the ill, injured, frail and impaired
- Comfortable addressing issues of death/dying
- Calm demeanor when entering the patients home
