Job Summary
Establish and maintain positive relationships with customers and referral sources within the community while developing access to Bristol Hospice Services. Complete hospice consults/evaluations, admissions, and initial patient/family assessments to determine care needs. Facilitate the referral process by coordinating communication with agency leadership, ED, DPCS, and physicians to ensure timely admission. Provide patient/family education related to the plan of care and utilize health assessment data to determine nursing diagnoses. Maintain comprehensive working knowledge of hospice resources, participate in Quality Assurance Performance Improvement processes, and function as an RN Case Manager with an assigned patient load. Travel 20% of the time with flexible work hours.
Required Qualifications
- Must have a current RN license in the state
- Must have a minimum of one (1) year of experience as a professional nurse within the last three (3) years or have a bachelor's degree in nursing from a program accredited by the National League of Nursing
- Must be able to travel 20% of the time, often without notice
- Must be flexible in work hours
- Must possess the willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

- An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill
- Ability to work autonomously with little direct supervision
- Successfully demonstrate tact, patience and good personal hygiene
- Ability to communicate effectively
- Strong organizational skills
- Understanding of plan of care documentation
- Flexible in work hours to ensure the patients receive care as provided in the plan of care
- Ability to empathize with the needs of the ill, injured, frail and impaired
- Comfortable addressing issues of death/dying
- Calm demeanor when entering the patients home
