Job Summary
Establish and maintain positive relationships with customers and referral sources within the community while developing access to Bristol Hospice Services. Complete hospice consults/evaluations and admissions, including initial assessments of patients and families to determine hospice needs. Provide professional nursing care by utilizing the nursing process, use health assessment data to determine nursing diagnoses, and facilitate the hospice referral process. Serve as a liaison between facility, physicians, hospitals, and community programs to coordinate referrals and admission services. Maintain comprehensive working knowledge in hospice educational resources and share information with organizational personnel. This role requires 20% travel and flexible work hours. Candidates must hold a current RN license with at least one year of recent professional nursing experience or a Bachelor's degree in nursing.
Required Qualifications
- current RN license in the state
- minimum of one (1) year of experience as a professional nurse within the last three (3) years
- bachelor's degree in nursing from a program accredited by the National League of Nursing
- ability to travel 20% of the time, often without notice
- flexible in work hours
- willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
