Admissions Registered Nurse (RN)

Conduct initial and ongoing patient assessments and assist in developing the Hospice Plan of Care. Educate patients and families on medication administration, dressing changes, and hospice philosophy while acting as the primary liaison between physicians, families, and the care team. Maintain accurate medical records and perform routine supervisory visits at patient residences or facilities. Participate in the agency on-call schedule, with weekly calls as needed, while adhering to all compliance guidelines and reporting suspected violations promptly. Requires a Louisiana RN license, CPR certification, EMR knowledge, and previous hospice experience.
