Job Summary
As the Home Health Admission RN in Colorado Springs, you will perform initial and ongoing comprehensive assessments, coordinate patient services, provide nursing care per the patient/clinical plan, and collaborate with patients and families to clarify goals and incorporate them into the plan of care. You will conduct home visits, develop care plans with the Case Manager, and ensure therapeutic, preventive, and rehabilitative nursing actions are implemented. The role requires an accredited nursing program graduate with an active Colorado RN license, at least 1 year of nursing experience, recent Home Health and Oasis experience, CPR certification, and ability to work independently. Travel time and mileage are reimbursed. Full-time position with on-call rotation, salary $90,000 per year, located in Colorado Springs, with benefits including medical/dental/vision, continuing education, and opportunity for growth.
Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in Colorado
- Minimum of one (1) year of nursing experience
- Recent Home Health and Oasis experience required
- CPR Certification (demonstration course)
- Pass a background check and drug test
- Able to work independently and communicate effectively with patient/family, team members, and other health care professionals
- Knowledge of state, federal, and licensure regulations
