Job Summary
Conduct initial patient assessments, collect comprehensive health histories, and coordinate with the interdisciplinary team to develop individualized care plans. Educate patients and families on treatment plans and healthcare procedures while ensuring accurate and timely documentation in compliance with regulations. Participate in discharge planning and patient follow-up as needed. This role requires a valid New Jersey RN license with at least two years of acute care experience, proficiency in EMR systems, and strong communication skills. Work shifts run from 12:00 PM to 8:30 PM with virtual interviews available. Required Qualifications
- Valid New Jersey RN license
