Job Summary
Initiate admission assessments for new residents and hospital returns while developing initial care plans. Coordinate the admission process with the interdisciplinary team, review and evaluate care plans as conditions warrant, and complete required charting, MDS, and other documents. Audit records to ensure compliance with facility standards and participate in discharge planning when necessary. Maintain confidentiality of resident information per HIPAA guidelines and may supervise resident care as assigned. Requires an active NYS RN license and prior admissions experience in long-term care.
Required Qualifications
- active / unrestricted NYS Registered Nurse License
- Prior experience as an admissions nurse in LTC settings
- excellent clinical assessment skills
- Ability to make good independent decisions
- Strong leadership and supervisory skills
- Computer savvy; proficient in MS Office (Excel, Word, Outlook)
- Excellent communication and interpersonal skills
Desired Qualifications
- Long-Term Care Experience Preferred
