Job Summary
Initiate admission assessments for new residents and hospital returns, coordinating the full admission process with the interdisciplinary team and developing initial care plans. Audit records to ensure compliance with facility standards, review and evaluate care plans, and complete MDS documentation and charting per policy. Assist with nursing department direction, participate in discharge planning when necessary, and supervise resident care as assigned while maintaining strict confidentiality. Requires an active NYS RN license, prior admissions experience in LTC, and strong clinical assessment and leadership skills. Located in Glens Falls, NY, with daily pay and premium Aetna health coverage.
Required Qualifications
- active / unrestricted NYS Registered Nurse License
- Prior experience as an admissions nurse in LTC settings
- excellent clinical assessment skills
- Ability to make good independent decisions
- Strong leadership and supervisory skills
- Computer savvy; proficient in MS Office (Excel, Word, Outlook)
- Excellent communication and interpersonal skills

- Long-Term Care Experience Preferred
