Job Summary
Conduct intake and case management activities for applicants and participants via home visits and telehealth to ensure care needs are met. Perform assessments, develop person-centered Plans of Treatment for HCBA Waiver, CCT, and ALW enrollments, and review medical records to verify appropriate health services and medications. Collaborate with interdisciplinary teams including social workers and HCBA case managers to update enrollment documents and coordinate monthly visits. Communicate with providers, patients, and housing representatives regarding care needs while documenting all interactions in the electronic health record. Address deferred cases with Department of Health Care Services Nurse Evaluators and institute quality indicator tracking to improve patient outcomes.
Required Qualifications
- Current California State license as a Registered Nurse
- Valid California Driver's License
- Eligible to be insured under our liability policy with a clean DMV report
- Your own vehicle and proof of current auto insurance
- Proficiency using software programs, and familiar with Microsoft office product including Word and Excel
- Ability to work independently and with others at all levels within the organization and collaborate effectively
- Above-average interpersonal, problem-solving, written and oral communication skills
- CPR certificate is required within 30 days of employment
- Ability to lift to 35 pounds without assistance
- Ability to move, visual acuity, and hearing acuity (as tested by medical exam)
Desired Qualifications
- Discharge planning, home health or hospice experience
- At least one year of work experience as an RN
- Inquisitive nature and analytical skills
- CCM certification
