Job Summary
Assess assigned case loads of home health care patients to identify physical, psychosocial, and environmental needs through documentation, clinical records, and on-site evaluations. Make initial evaluations and re-evaluate nursing needs during each visit, then coordinate, implement, and evaluate the plan of treatment while revising it based on identified care issues. Communicate significant findings, problems, and changes in health status or unsafe facility conditions to the Clinical Supervisor, physician, and other personnel. Requires current unrestricted state RN license, one year of home health experience, and ability to travel 50% within the Haverhill territory.
Required Qualifications
- Current, unrestricted state RN licensee
- 1+ year experience in a Home Health setting as a nurse
- Able and willing to travel within branch/office coverage area
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must have reliable transportation, valid driver's license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
Desired Qualifications
- Associates Degree or Bachelor's Degree in Nursing is preferred
- Previous experience with HomeCare HomeBase is preferred
